E-Sign Consent

To apply and manage your plan online, we need your consent to provide records and signatures electronically, as permitted by the federal Electronic Signatures in Global and National Commerce Act (E-SIGN) and Florida’s Uniform Electronic Transaction Act.

What this consent covers

Applications, disclosures (including Truth-in-Lending and commercial financing disclosures), your financing agreement, notices of action taken, privacy notices, payment receipts, past-due and device-protection notices, and other communications about your plan.

How we deliver records

We post records in your online account and/or email them to the address on file. Keep your email address current in your profile.

Paper copies and withdrawing consent

You may request a paper copy of any record at no charge, or withdraw your consent to electronic records, by contacting us at (772) 356-3550, support@eburstservices.com, or eBurst Services Inc, 1860 SW Fountainview Blvd, Ste #1045, Port St. Lucie, FL 34986. Withdrawing consent doesn’t affect records already provided, and we may need to close online access to an application in progress.
